Our patients now have the ability to pre-register online. We want to make preparing for your hospital visit as simple and stress-free as possible, whether you are coming to give birth or get a mammogram.

Who Can Pre-Register?

• Patients with a procedure already scheduled
• OB/Maternity patients who would like to pre-register for Labor and Delivery
• Patients who need to request a screening mammography appointment

Please pre-register online at least 48 hours before your visit to the hospital to allow adequate time for processing your information.

What you'll need to Pre-Register:

• Your personal contact information including occupation information
• Emergency contact information
• Insurance information, group name and number, policy number, address and telephone number
• Physician name
• Date of procedure

You may be required to sign the following

• Consent for Treatment
• Authorization to bill insurance
• Important Message from Medicare, if applicable

Also, as a Medicare patient, we are required to ask 

• Medicare Secondary Payer Questions

Remember to bring with you at the time of registration the following items:

• Picture ID
• All active insurance cards
• Complete list of current Medications
• Living Will, or Advance Directive if you have one

Patients are encouraged to leave valuables at home to ensure safe keeping.

Upon arrival, go the Registration Office located in the front Lobby of the hospital to register. It is important that you arrive on time, because your physician may have scheduled special tests for you. We are required by law to ask if you have an Advance Directive.
